Information We Collect
SVMD and its third-party service providers collect and store certain personal  information that you provide or otherwise convey through your use of this Site, such as web pages that allow you to provide us with input on this Site, participate in on-line surveys, apply for open positions or correspond with certain departments within the hospital (such as the billing department). Such information may include your name, address, e-mail address, social security number, credit card information (when applicable) as an application to your specific request. Minors

Legal

California Civil Code sections 1798.83-1798.84 give California residents the right to ask SVMD for a notice describing the categories of personal  information it shares with third parties or corporate affiliates for their direct marketing purposes. The notice will identify the categories of information shared and will include a list of the third parties and corporate affiliates with which it has been shared, along with their names and addresses. Retention
We will hold the personal information we collect about you for as long as is necessary to provide services to you. Please submit requests to questions@svmdmed.org.

Changes or Deletion
If you would like to update, correct or have personal information removed from this Site, please contact us at questions@svmdmed.org with your request, and we will use reasonable efforts to comply with your request. Upon receipt of a request to update, correct or delete personal information, we may ask you to put your request in writing, provide proof of your identity, and specific scope of your request. If we determine that we do not have enough information about you or your request to carry out the correction or deletion, we will give you our reasons for the decision. We may require you to pay a reasonable fee for such service. Deletion of the applicable personal information may prevent you from using certain functions on this Site and such actions will not affect prior stored access logs and records, which may or may not contain such personal information. We may sell or transfer assets or portions of our business as we continue to improve our product and service offerings and you consent to the transfer of your personal information as one of the transferred business assets.
