Information We Collect

Legal

Retention
We will hold the personal information we collect about you for as long as is necessary to provide services to you. Please submit requests to questions@svmdmed.org.
Changes or Deletion
If you would like to update, correct or have personal information removed from this Site, please contact us at questions@svmdmed.org with your request, and we will use reasonable efforts to comply with your request. Upon receipt of a request to update, correct or delete personal information, we may ask you to put your request in writing, provide proof of your identity, and specific scope of your request. If we determine that we do not have enough information about you or your request to carry out the correction or deletion, we will give you our reasons for the decision. We may require you to pay a reasonable fee for such service. Deletion of the applicable personal information may prevent you from using certain functions on this Site and such actions will not affect prior stored access logs and records, which may or may not contain such personal information.
